We encourage you to complete the patient registration forms by downloading them prior to scheduling your appointment. This will expedite your visit and reduce the amount of time you spend in the waiting area. Once the forms are complete, you can email them to  patientinfo@cardiofitla.com or bring them to the office.

Please remember to bring your insurance card with you to your visit.

Appointments

Appointments can be made by calling our office at (310)-791-5577.

Appointment Cancelation Policy

Because many of our new patient appointments require large blocks of time and, in some cases, staff may be required to come in early to prepare, we have instituted an appointment cancelation policy. Appointment cancelations must be made at least 24 hours in advance. Otherwise, a $75.00 fee will be assessed to the patient. for canceling your appointment without 24 hours or more notice. We appreciate your understanding. You will be asked to leave a credit card on file when you schedule your first appointment.

Prescription Refills

When you need a prescription refilled, please call our office at least 2-3 days prior to running out. This gives the physicians time to review your chart and approve the refill.

When calling, please provide us with your pharmacy’s telephone number and the name of the medication.

We do not refill medications after hours.

Friday: 8am – 12pm

Insurance and Payment

We accept most major insurance plans. Click here for a complete listing. Co-payments and deductibles are due at the time of service. We accept major credit cards, checks and cash.
